Community Worker

Access Disability Support Services is a registered NDIS provider delivering disability and community-based services across Western Australia. We are looking for two compassionate and experienced Community Workers to join our growing team in full-time, permanent positions.

In this role, you will work closely with participants, families, service providers and community organisations to help people access suitable services, build independence and participate meaningfully in their communities.

Key responsibilities

1. Assess participants’ needs, strengths, goals and barriers to community participation.
2. Develop and implement individual community support plans in consultation with participants, families and relevant professionals.
3. Connect participants with suitable community programs, health services, accommodation, education, employment and social-support networks.
4. Provide information, advocacy and practical assistance to help participants understand and access available community services.
5. Liaise with families, allied health professionals, support coordinators, government agencies and community organisations to coordinate appropriate referrals and services.
6. Organise and facilitate community activities, social participation programs, workshops and capacity-building initiatives.
7. Monitor participants’ progress, identify changes in their circumstances and adjust support strategies where required.
8. Maintain accurate case notes, progress reports, referral records, service documentation and other required participant files.

Skills and personal qualities

The successful applicants will be compassionate, patient and respectful, with a genuine commitment to supporting people with disability. You will have strong communication and interpersonal skills, sound judgement and the ability to build trust with participants from diverse cultural and social backgrounds.

You must also be able to prepare clear file notes, progress reports and referral documentation, communicate professionally with families and service providers, manage sensitive information confidentially and work both independently and as part of a multidisciplinary team.

Essential requirements

1. A minimum Diploma of Community Services or a closely related qualification.
2. At least two years of relevant experience with an NDIS provider, community-services organisation, disability service or allied health provider.
3. A valid Western Australian driver’s licence.
4. A reliable and insured vehicle suitable for work-related travel.
5. A current National Police Check.
6. A current Working With Children Check or willingness to obtain one.
7. A current NDIS Worker Screening Check or willingness to obtain one.
8. Current First Aid and CPR certification or willingness to obtain it before commencing employment.
